Demographics: 25 years old, Female
Indication: Acute dyspnea

Case #2

Findings

  • Large right pneumothorax
  • Mild leftward mediastinal shift
  • Bandlike opacity in the right lower lung
  • Left subclavian approach single lumen port-catheter with catheter tip overlying the superior cavoatrial junction
  • Partially imaged left double-J ureteral stent with proximal loop overlying the expected location of the left renal pelvis or proximal ureter

Diagnosis

Tension pneumothorax

Sample Report

Large right pneumothorax with associated partial right lung atelectasis and leftward mediastinal shift. Recommend correlation with clinical signs of tension physiology.

Left lung is clear without evidence of pneumonia or pulmonary edema.

No pleural effusions.

Discussion

  • Remember that tension is a clinical diagnosis, but make sure to alert the referring clinician to the possibility of tension when you see mediastinal shift
