Case Filters, Smarter AI & Platform Polish

v2.4
โฌ†๏ธ Improvement

A big roundup: filters for your saved cases and the call simulator, a more capable AI model with steadier differentials, faster DICOM loading, translated annotation labels, a new Charity page, a public CME page, and a long list of login, localization, and case fixes.

Saved Cases & Call Simulator

  • Filter Saved Cases by Topic โ€” The Cases tab on your Saved Items page now has filter chips for modality (CT, X-Ray, MRI, Ultrasound, Nuclear Medicine), body region (Chest, Abdomen/Pelvis, Neurological, MSK, General), and adult vs peds. Pick "Neurological" + "Peds" and you'll see only your saved peds neuro cases. Chips combine.
  • Age Group Filter on the Call Simulator โ€” Added an Age Group option (All ages / Adults / Peds) in the call simulator's Other Options. Combined with the topic-selector at the top of the setup form, you can now build a peds neuro, peds ultrasound, or adults-only call simulator in two clicks.

DICOM Viewer

  • Faster Loading on Large Multi-Series Cases โ€” Body CT, MRI spine, and other cases with many series now load noticeably faster, and the loading progress bar advances reliably even on heavily loaded browser sessions.
  • Faster DICOM Loading in More Regions โ€” Added regional DICOM servers for South America and Australia/NZ so cases load faster for users in those areas.
  • Translated Annotation Labels โ€” Annotation popups and on-image labels in the DICOM viewer now appear in your selected language alongside the rest of the translated case content.

AI Tools

  • Smarter AI for Voxel Chat & Differential Assistant โ€” Voxel Chat and the Differential Assistant are now running on a more capable AI model under the hood. Expect stronger reasoning on complex cases and richer back-and-forth in chat.
  • Steadier Differential Answers โ€” Fixed cases where the Differential Assistant could return a blank or cut-off response on complex cases.
  • Voxel Chat Accepts Clinical Phrasing โ€” Typing phrases like "onset = acute" no longer trips a false "invalid content" warning in Voxel Chat.

Call Prep & Cases

  • Accurate Completion Counts Across Languages โ€” Completing a case now counts once no matter which language you studied it in; dashboard and topic completion totals are corrected.
  • Progress Bars on Translated Pages โ€” Call-prep progress bars now display correctly on non-English topic pages.

Anatomy Viewer

  • Cardiac Module Corrected โ€” The cardiac anatomy module now uses CT (it was previously mislabeled as MRI).

Account & Login

  • Verification Code as a Backup โ€” Email verification now includes a setup code alongside the link, with clearer feedback and a short per-email cooldown so you're not spammed and know when to check your inbox.
  • Fixed an SSO Login Loop โ€” Users signing in through certain hospital single-sign-on systems were getting bounced between login and logout; sign-in is now stable for those institutions.

International

  • Local Checkout & Tax in More Countries โ€” More countries can now check out with local payment and tax handling.

CME

  • Public CME Page โ€” The CME page is now viewable without logging in, with full program info.

Charity

  • New Charity Page โ€” Added a Charity page covering our RAD-AID partnerships (including RAD-AID Brazil).

Fixes

  • Contact Form Works Again โ€” Fixed an issue that prevented contact-form messages (including screenshots) from sending.

Quality of Life Improvements

v2.3
โฌ†๏ธ Improvement

A focused release: a new DICOM Viewer settings panel, a major localization push, smarter login throttling, higher AI usage caps, and a long list of bug fixes and small refinements.

Anatomy Viewer

  • Scroll Wheel Sensitivity โ€” New per-user sensitivity slider in the bottom bar. macOS trackpads no longer feel hyper-sensitive; the setting persists across sessions and applies to every anatomy module.

DICOM Viewer

  • Pick Your Default Tool โ€” New gear icon in the DICOM toolbar opens a settings panel where you can choose which tool is active when a case first loads (Window/Level or Scroll), flip the scroll direction, and tune scroll and W/L sensitivity. The same options also live in the footer settings menu and your account settings.
  • W/L Sensitivity Syncs Across Devices โ€” Window/Level sensitivity now persists to your account instead of just the current browser, so the value follows you between sessions and devices.
  • CT ROI Tool Stays Active โ€” Fixed an issue where the CT ROI tool was disabled after first use.
  • CT Measurement Dropdown โ€” Re-selecting the same measurement tool from the dropdown now activates it correctly.
  • Always-Visible Toolbar Labels โ€” W/L and Scroll labels are now always shown in the DICOM toolbar.
  • Cleaner Mobile Toolbar โ€” Fullscreen, rotate, and invert tools are now hidden on small screens to reduce clutter.
  • No More Accidental Fullscreen Exits on Mobile โ€” On iOS Safari and other touch devices, scrolling or swiping within a fullscreen DICOM viewer no longer kicks you out of fullscreen. Especially helpful while reporting a case, where the old behavior forced you to re-open fullscreen repeatedly.
  • Smoother Session Handling โ€” Expired sessions during DICOM viewing are handled gracefully without unexpected logouts.

AI Tools

  • Higher Grading Limits โ€” PRO monthly assessment and call sim grading allowance raised from 300 โ†’ 500/mo.
  • Higher Dictation Limits โ€” PRO monthly speech-to-text dictation cap raised from 150 โ†’ 400 minutes for users doing heavier reporting workflows.
  • Better Detection of Silent Grading Failures โ€” Added observability so silent AI grading failures surface to the team faster.
  • AI Boost Add-on (Coming Soon) โ€” Working on an optional add-on that lifts AI usage limits even further across every AI tool (dictation, read-aloud, Voxel Tutor, impressions, differentials, and more) for power users.

Subscriptions & Billing

  • All Active Subscriptions on Billing Tab โ€” The Account โ†’ Billing tab now lists every active subscription, not just the most recent.
  • Multi-Subscription Renewals โ€” Users with more than one active subscription can now renew an expiring plan without it conflicting with their other subscriptions.

Account & Login

  • Smarter Login Rate Limiting โ€” Login throttling now distinguishes between individual users on shared networks, so legitimate users on offices, hospitals, or shared Wi-Fi are no longer locked out because someone else nearby mistyped a password.

Search & Public Pages

  • Guest Search Works Again โ€” Logged-out visitors searching the site now get the public results page instead of a 403.

Translated Cases

  • Media + Course Pages Restored โ€” Fixed an issue where translated cases were missing media files and dropping their course-page children. Translated cases now render with full content parity.

Localization

  • Major Localization Push โ€” Translated remaining UI surfaces across all supported languages: pricing page, homepage, mobile menu, call simulator, header toolbar, DICOM viewer, Voxel chat, AI disclaimer, cookie consent banner, account settings, search page, and structured data. Also fixed sticky-language and locale cache-poisoning bugs, query-string preservation across language redirects, and added an English-authoritative disclaimer on translated legal docs.

CME, Programs & Cases

  • Reliable CME Credit Awarding โ€” Fixed a rare issue where CME credits could fail to award silently during high concurrency. Any affected credits have been backfilled.
  • Neuro Fellowship Access Fix โ€” Corrected the program access level granted to Neuro Fellowship members.
  • View Case Details Link โ€” Fixed for cases outside the assessments collection.

Greetings & Leaderboards

  • More Greeting Variations โ€” Added new rotating greetings on the dashboard, including a dedicated "Weekend is here" greeting on Saturdays and Sundays.
  • All Period Data on Leaderboards โ€” Leaderboard updates now display all period data (daily/weekly/monthly), not just the active window.
  • Larger Avatars + Logos โ€” Bumped avatar and logo sizes in the leaderboard and dashboard greeting for better visibility.

Email & Notifications

  • Email Preferences Tab โ€” New dedicated tab in Account settings for managing digest and notification toggles.
  • Weekly Progress Digest is now opt-in โ€” The Monday morning recap of cases completed, badges earned, CME credits, and streaks no longer goes out by default. Users who want it can enable it in Account โ†’ Email Preferences.

Help & Documentation

  • Trimmed Help Articles โ€” Cut "Tips" and "Troubleshooting" filler from several help articles (anatomy viewer, lofi player, referral program, others) so they get to the point faster.
  • Sidebar Cleanup โ€” Reorganized help sidebar links for clarity and consistency.

  • Spine Level Labeling โ€” New measurement-menu presets for cervical, thoracic, and lumbar spine: each click drops the next vertebral label in sequence (e.g. S1 โ†’ L5 โ†’ L4โ€ฆ), and clicking on an existing label cycles it.
  • Annotations Across Languages โ€” Annotations on a case now carry through to its translated versions automatically.

Keyboard Shortcuts

  • Mac Shortcuts Fixed โ€” Option-key shortcuts (W/L, zoom, save, next case, etc.) now work correctly on Mac, where they were silently broken before. The shortcuts popup shows Mac symbols (โŒ˜ โŒฅ โ‡ง) in Apple's native style, and Cmd+S works as a save shortcut that survives typing inside text fields.
  • Cleaner Shortcuts Popup โ€” Reorganized into Case Navigation and Case Actions sections, with platform-aware modifier display and no duplicate entries for cross-platform shortcut pairs.
  • Unified Case Navigation โ€” Alt+โ†’ and Alt+โ† (โŒฅโ†’ / โŒฅโ† on Mac) now move to the next/previous case in both regular cases and the call simulator, replacing the older Alt+J / Alt+I.
  • Delete Key Fixed โ€” Pressing Delete on the DICOM viewer now removes the most recently drawn measurement (previously a silent no-op).
  • Better Mac Text Editing โ€” Shortcuts no longer interfere with typing accented characters on Mac (Option+E for รฉ, etc.) or with IME input for Chinese, Japanese, or Korean.

Text-to-Speech

  • Much Faster Playback โ€” Audio for call prep cases is now pre-generated so it plays back almost instantly instead of waiting several seconds to load.

CaseStacks 2.0 — Platform Relaunch

v2.0.0
๐ŸŽ‰ New Feature

We've rebuilt CaseStacks from the ground up. The entire platform has been migrated to a modern tech stack, delivering significant improvements in performance, security, and maintainability โ€” and laying the foundation for the features below.

Advanced DICOM Viewer

Our new DICOM viewer is a major leap forward with powerful tools for interacting with medical imaging.

  • Measurement Tools โ€” Length, angle, and bidirectional measurement tools let you quantify findings directly on the image.
  • Region of Interest (ROI) โ€” Draw elliptical or rectangular ROIs with automatic Hounsfield Unit calculations.
  • Linked Annotations โ€” Click any annotation referenced in the case findings and the viewer automatically navigates to that exact location on the image.
  • W/L Presets โ€” One-click windowing presets for soft tissue, lung, bone, brain, subdural, and stroke windows.
  • Custom Sensitivity Controls โ€” Fine-tune scroll speed and window/level drag sensitivity to your preference. Settings are saved to your account and persist across sessions.
  • Improved Series Performance โ€” Optimized loading with two-phase prefetching (priority images first, then full stack), memory management, and circuit-breaker fault tolerance for large series.

Study Tools

New tools built directly into the case viewing experience to support focused, uninterrupted study sessions.

  • Text-to-Speech โ€” Have case write-ups read aloud with high-quality AI voices. Choose from 8 voices, 4 playback speeds, and read individual sections or the full case. Great for auditory learners or reviewing on the go.
  • Notes โ€” Take private, rich-text notes on any case. Notes are saved to your account and linked to the case for easy reference later.
  • Pomodoro Timer โ€” A built-in study timer using the Pomodoro Technique with customizable work and break intervals. Completed sessions are tracked in your activity history.
  • Lo-fi Music Player โ€” Curated lo-fi playlists you can listen to while studying โ€” no need to leave the site.

AI Tools (Beta)

A suite of AI-powered assistants designed to support radiology education. All tools include safety measures and are grounded in evidence-based guidelines where applicable.

  • Voxel โ€” An AI chat tutor that lives in a sidebar panel. Ask questions, discuss cases, or get help understanding findings. Voxel can search PubMed for peer-reviewed literature and look up incidental findings guidelines mid-conversation. Adapts responses to your training level.
  • Impression Generator โ€” Paste your findings and clinical history to generate a structured radiology impression. Supports multiple report styles and modalities. Can also flag potential incidental findings with guideline-based recommendations.
  • Differential Assistant โ€” Enter imaging findings to generate an evidence-based differential diagnosis. Uses real-time web search grounding to provide current literature citations. Supports a clarifying-questions workflow to refine results, and you can save differentials for later reference.
  • Incidental Findings Assistant โ€” A deterministic lookup tool (not AI-generated) for evidence-based follow-up recommendations. Covers ACR and Fleischner guidelines for pulmonary nodules, thyroid nodules, adrenal nodules, liver lesions, adnexal findings, and gallbladder polyps. Returns exact citations, follow-up intervals, and clinical caveats every time.

Redesigned Call Simulator

The call simulator has been overhauled from a simple random-shuffle format into a structured, session-based experience.

  • Sessions โ€” Cases are now organized into discrete sessions rather than an endless random shuffle. You can configure each session before starting and review your performance afterward.
  • Time & Case Limits โ€” Set a time limit (up to 3 hours) or a case limit (up to 50 cases) per session, or run unlimited. Time-limited sessions show a live countdown.
  • Resume Prior Sessions โ€” Leave a session and pick up exactly where you left off. Active sessions are detected automatically when you return.
  • Distractions Mode โ€” Simulate the reality of a call shift. Enable distractions and the simulator will interrupt you with realistic scenarios โ€” ED physicians calling about emergencies, ICU nurses requesting urgent reads, techs asking protocol questions. Choose from quiet, moderate, or busy frequency. You respond in real time, and your interactions are saved with the session.
  • AI-Graded Preliminary Reports โ€” Write a preliminary report for each case just like you would on a real call shift. After submission, an AI grading engine evaluates your report on a 0โ€“10 scale against the expert findings and diagnosis. You receive detailed feedback highlighting what you identified correctly, what you missed, and educational context explaining why those findings matter. The grading rubric prioritizes diagnostic accuracy first, then finding identification and report quality.

Revamped Assessments

Assessments have been rebuilt with the same session-based structure and AI grading engine used in the call simulator, creating a more rigorous and flexible evaluation experience.

  • Session-Based Structure โ€” Assessments are now organized into configurable sessions. Set a time limit, case limit, or run unlimited. Name your sessions for easy reference later.
  • AI-Graded Reports โ€” Submit a preliminary report for each case and receive AI-powered feedback using the same grading rubric as the call simulator. Reports are scored on a 0โ€“10 scale with detailed, educational feedback.
  • Grade Display Options โ€” Choose to see your grade and feedback after each case, or wait until the end of the session to review all results together.
  • Modality Filtering โ€” Filter available cases by imaging modality (CT, MRI, radiograph, ultrasound, nuclear medicine) when configuring a session.
  • Case Locking โ€” Once a case is graded in an assessment session, it's locked to preserve the integrity of your score. You can still attempt new cases.
  • Performance Analytics โ€” Track your average scores, score distributions, and progress over time. Program administrators can view aggregate statistics across their residents.

Multilanguage Support

CaseStacks is now available in 9 languages:

English, German, French, Spanish, Japanese, Portuguese, Chinese, Italian, and Korean.

The interface, navigation, dashboard, settings, and feature descriptions are fully translated. Select your preferred language from the language selector and your choice is saved to your account.

Social & Gamification

New features to keep you motivated and connected to the CaseStacks community.

  • Leaderboard โ€” See who's completing the most cases and who holds the longest study streaks. Leaderboards also rank radiology programs by total cases completed. Privacy controls let you opt out if you prefer.
  • Badges โ€” Earn badges for milestones and achievements across 7 categories: progress (cases and quizzes completed), streaks (consistent daily study), performance (perfect scores), specialty mastery (neuro, chest, MSK), engagement (call simulator, forum activity, saved items), and more. Showcase up to 5 favorite badges on your profile. 30 badges to earn in total, including 2 secret badges.
  • Streaks โ€” Track your daily study consistency. Hit 7 days for Week Warrior, 30 for Consistent Learner, and 100 for Dedicated Scholar.

Enhanced Progress Page

The progress page has been redesigned with interactive charts and visualizations to give you a clearer picture of your learning.

  • Daily Progress Chart โ€” Track your case completions over time with configurable views (7, 30, 90 days, or all-time).
  • Category Breakdown โ€” See your completion rates across course categories at a glance with doughnut and bar charts.
  • Activity Heatmap โ€” Visualize your study patterns across days of the week and hours of the day over the past 12 weeks.
  • Session Statistics โ€” Review your call simulator and assessment performance including cases reviewed, reports written, average grades, and time spent.
