Findings
CT
- Hyperattenuating mass centered in the left cerebellar hemisphere and vermis with adjacent parenchymal edema
- Small cystic components
- Associated obstructive hydrocephalus at the level of the fourth ventricle with subependymal edema
MRI
- Solid mass with small cystic components centered in the left cerebellar hemisphere and vermis, extending inferiorly into the posterior aspect of the foramen magnum. The mass measures 5.5 x 3.5 x 4 cm
- The solid components of the mass avidly enhance and demonstrate restricted diffusion
- Surrounding parenchymal edema and local mass effect with effacement of the fourth ventricle and obstructive hydrocephalus with subependymal edema
Diagnosis
Medulloblastoma (likely SHH-activated, TP53 wild-type)
