Findings
CT
- Extensive vasogenic edema in the left greater than right cerebral hemispheres with a posterior predominance
- Associated mass effect with crowding of the left lateral ventricle and slight left-to-right midline shift
- No evidence of downward transtentorial herniation or hydrocephalus
MRI
- Focus of restricted diffusion in the left frontal periventricular white matter, consistent with an acute/early subacute ischemic infarct
- Extensive T2/FLAIR signal hyperintensity in the left greater than right cerebral white matter with a posterior predominance. Additional involvement of the left parietal cortex
- Numerous intermixed foci of parenchymal susceptibility artifact in a predominantly lobar distribution with relative sparing of the deep gray structures
- No corresponding enhancement
