Findings

CT

  • Mildly hyperattenuating pineal region mass with a cystic component along its right margin
  • Pineal calcification is displaced along the superior aspect of the mass
  • Associated obstructive hydrocephalus at the level of the cerebral aqueduct

MRI

  • Heterogeneously-enhancing pineal region mass measuring 2 x 2 x 2.1 cm with extension into the right greater than left thalami and midbrain tectum
  • Mild patchy corresponding restricted diffusion
  • Small internal cystic components eccentric to the right
  • Associated obstructive hydrocephalus at the level of the cerebral aqueduct
