Updated CME Claim Fees

We've simplified the pricing for CME claim fees, as detailed below. This change will allow us to add additional courses without making pricing overly complicated. For existing users this is unlikely to result in an increase in cost for a claim, but if it does we are happy to honor the old price. Please contact us if you have a question.
