Findings
- Numerous supratentorial and infratentorial periventricular and juxtacortical T2/FLAIR hyperintense lesions including extensive lesions throughout the brainstem, many of which have associated T1 hypointensity
- Many lesions are oriented perpendicular to the ventricular margins
- Incomplete ring enhancement associated with lesions in the left cerebellar hemisphere, posterior left temporal lobe, left frontal lobe, and right cingulate gyrus
- No associated restricted diffusion
- No evidence of acute infarct, hemorrhage, mass effect, or hydrocephalus
- Severe mucosal thickening of the left maxillary sinus
Diagnosis
Multiple sclerosis with active demyelination
Sample Report
Extensive supratentorial and infratentorial white matter lesions with appearance and distribution consistent with the reported history of multiple sclerosis.
Several lesions in the left cerebellar hemisphere, left frontal and temporal lobes, and right cingulate gyrus have associated enhancement concerning for active demyelination.
Left maxillary sinusitis.
