Findings
- Numerous foci of susceptibility artifact in the bilateral frontal juxtacortical white matter, bilateral temporal lobes, left subinsular region, left eccentric body and splenium of the corpus callosum, left caudate body, right greater than left globus pallidus, bilateral thalami, left dorsal and right ventral midbrain, posterior pontomesencephalic junction, and right cerebellar hemisphere
- FLAIR hyperintense signal in the interpeduncular cistern
- Susceptibility artifact layering in the occipital horns of both lateral ventricles
- Faint restricted diffusion in the splenium of the corpus callosum, more conspicuous on the ADC map
Diagnosis
Diffuse axonal injury (DAI)
Sample Report
Severe diffuse axonal injury evidenced by microhemorrhages in the bilateral frontotemporal juxtacortical white matter, deep gray nuclei, thalami, brainstem, and right cerebellar hemisphere.
Trace subarachnoid hemorrhage layering in the interpeduncular cistern as well as blood products layering in the occipital horns of both lateral ventricles. No hydrocephalus.
Faint restricted diffusion in the splenium of the corpus callosum, more conspicuous on the ADC map, which could also relate to diffuse axonal injury; however, recommend correlation with evidence of seizure activity.
