Findings

  • Moderate-sized right pleural effusion which freely layers on the right-side-down decubitus view
  • Hazy opacification in the right lung base
  • No pneumothorax
  • Mild enlargement of the cardiopericardial silhouette
  • Median sternotomy changes

Diagnosis

Simple pleural effusion

Sample Report

Moderate-sized right pleural effusion which freely layers on the right-side-down decubitus view.

Hazy right basilar opacification is favored to represent a combination of layering pleural fluid and atelectasis.

Discussion

  • In most cases, place the side you want to evaluate down when doing decubitus views to determine whether or not a pleural effusion is loculated. Occasionally, however, the opposite decubitus view can also provide useful information
