Demographics: 82 years old, Male
Indication: Dyspnea, lung cancer undergoing radiation therapy

Findings

Chest

  • Spiculated mass in the right upper lobe measuring 4.5 x 2.5 x 2 cm with relative central hypoattenuation
  • Surrounding architectural distortion
  • Mild centrilobular and paraseptal emphysema
  • Mild central bronchial wall thickening
  • Filling defect in the left atrial appendage

Upper abdomen

  • 3 x 2.5 cm low density structure in the interpolar left kidney, likely a cyst
  • Additional bilateral renal subcentimeter hypodensities are too small to characterize
  • Wedge-shaped areas of hypoattenuation in both kidneys, which may represent sequela of prior infarcts

MSK

  • No acute findings

Diagnosis

Left atrial thrombus

Lung cancer

Sample Report

Spiculated right upper lobe mass consistent with reported history of lung cancer. Surrounding architectural distortion could relate to the neoplasm itself or developing postradiation changes.

Filling defect in the left atrial appendage concerning for thrombus.
