Demographics: 44 years old, Female
Indication: Dyspnea not improving after antibiotics

Findings

Chest radiograph

  • No focal airspace opacification or edema
  • Aortic atherosclerosis

CT

Chest

  • Multifocal bilateral peripheral consolidation and groundglass opacities
  • Several opacities demonstrate relative central hypoattenuation
  • Mild central bronchial wall thickening
  • Conspicuous number of nonenlarged mediastinal and bilateral hilar lymph nodes
  • Thoracic aortic atherosclerosis without aneurysm

Upper abdomen

  • No acute findings
  • Cholecystectomy

MSK

  • No acute findings
  • T8 superior endplate Schmorl's node

Diagnosis

Cryptogenic organizing pneumonia

Sample Report

Multifocal bilateral peripheral airspace disease which could relate to atypical infection. However, given reported lack of improvement on antibiotics, cryptogenic organizing pneumonia is an additional consideration and could have this imaging appearance. Recommend followup chest CT after treatment to ensure resolution.
