Demographics: 62 years old, Male
Indication: Cough, night sweats

Findings

Chest radiograph

  • Bilateral apical cavitary lesions with scattered calcifications and thick, irregular walls extending to the pleural surfaces
  • 8 mm soft tissue density structure within the area of cavitation in the left upper lobe
  • Upper lobe predominant moderate centrilobular and paraseptal emphysema
  • Noncalcified 6 mm nodule in the right lower lobe. Additional scattered calcified granulomas
  • Thoracic aortic atherosclerosis without aneurysm
  • Heavy coronary artery calcification

Upper abdomen

  • No acute findings

MSK

  • No acute findings

Diagnosis

Pulmonary tuberculosis

Sample Report

Bilateral apical cavitary lesions, which given the clinical history are most concerning for reactivation tuberculosis. Recommend correlation with laboratory analysis.

A small soft tissue nodule within the left upper lobe cavitary lesion may represent a superimposed fungus ball.

Indeterminate 6 mm right lower lobe nodule. Recommend follow-up chest CT in 12 months for further evaluation.
