Demographics: 60 years old, Male
Indication: Generalized abdominal pain

Findings

Lower chest

Abdomen/Pelvis

  • Gallbladder is distended with wall thickening, multiple calcified gallstones, pericholecystic fat stranding, and nonloculated fluid along the inferior aspect of the gallbladder neck
  • Discontinuous mural enhancement along anterior and posterior margins of the gallbladder
  • Multiple mildly enlarged periportal lymph nodes, likely reactive
  • Mild splenomegaly
  • Persistent left IVC which drains into the left renal vein. Normal right suprarenal IVC
  • Nonspecific cystic changes in the seminal vesicles

MSK

  • No acute findings
  • Median sternotomy changes
  • Multilevel degenerative changes of the spine

Diagnosis

Acute gangrenous cholecystitis

Sample Report

Findings concerning for acute gangrenous cholecystitis. Recommend surgical evaluation.

Recommend nonemergent chest CT to further evaluate a 7 mm left lower lobe pulmonary nodule and 10 mm subcarinal lymph node.
