Hallazgos
- Restricted diffusion and T2/FLAIR signal hyperintensity involving the bilateral corpus striatum as well as patchy involvement of the cerebral cortex bilaterally, particularly in the left frontal lobe and left cingulate gyrus
- Additional faint restricted diffusion involving the bilateral thalami
- Dilated perivascular space in the left putamen
- Mild mucosal thickening of bilateral ethmoid air cells

Restricted diffusion and T2/FLAIR signal hyperintensity involving the bilateral corpus striatum as well as patchy involvement of the cerebral cortex bilaterally, particularly in the left frontal lobe and left cingulate gyrus. Additional faint restricted diffusion involving the bilateral thalami. This appearance raises particular concern for Creutzfeldt-Jakob disease (CJD) in the clinical context of progressive neurologic decline.
