- Expansile, destructive lesion in the proximal fibular diaphysis measuring 8 cm in craniocaudal extent with wide zone of transition, aggressive "hair-on-end" periosteal reaction, and soft tissue extension
- No acute fracture or malalignment
Diagnosis
Ewing sarcoma

Expansile, destructive lesion in the proximal fibular diaphysis measuring 8 cm in craniocaudal extent with wide zone of transition, aggressive "hair-on-end" periosteal reaction, and soft tissue extension. While this could represent osteomyelitis in the correct clinical setting, an aggressive neoplasm such as osteosarcoma, Ewing sarcoma, or lymphoma could also have this appearance. Recommend MRI or soft tissue sampling for further evaluation.
No acute fracture or malalignment.
