Demographics: 8 years old, Female
Indication: Bilateral leg weakness

Findings

  • Large heterogeneously enhancing expansile intramedullary mass measuring up to 15 mm in anteroposterior diameter spanning C2-T6 with solidly enhancing and cystic peripherally-enhancing components
  • Central cord T2/STIR signal hyperintensity without corresponding enhancement extending inferiorly from the level of T6
  • Normal marrow signal
  • Normal alignment

Diagnosis

Spinal astrocytoma

Sample Report

Large heterogeneously enhancing expansile intramedullary mass measuring up to 15 mm in anteroposterior diameter spanning C2-T6 with solidly enhancing and cystic peripherally-enhancing components. This appearance is consistent with a spinal cord neoplasm, with primary differential considerations being astrocytoma and ependymoma. Nonenhancing central cord T2/STIR signal hyperintensity without corresponding enhancement extending inferiorly from the level of T6 likely represents a secondary syrinx. Recommend spine surgery consultation.
