Findings
- Diffuse ventriculomegaly, particularly involving the lateral and third ventricles with subtle undulations along the margins of the lateral ventricles
- Disproportionate enlargement of the Sylvian fissures with mild crowding of sulci in the high paramedian frontal and parietal lobes
- Evans index = 0.35
- Callosal angle = 68 degrees
- Patchy T2/FLAIR hyperintensities in the subcortical and periventricular white matter
- Generalized cerebral volume loss
- No evidence of acute infarct, hemorrhage, or mass effect
- Left pseudophakia

Normal pressure hydrocephalus (NPH)

Diffuse ventriculomegaly, with configuration of the ventricles and extraaxial CSF spaces typical for normal pressure hydrocephalus (NPH).
No evidence of acute intracranial abnormality.
Background of chronic small vessel disease and cerebral atrophy.
