Demographics: 89 years old, Female
Indication: Obtunded

Findings

  • Areas of restricted diffusion and T2/FLAIR hyperintensity in the right superior cerebellar hemisphere, left greater than right midbrain, and bilateral medial thalami
  • No evidence of acute hemorrhage, mass effect, or hydrocephalus
  • Remote infarcts in the bilateral cerebellar hemispheres
  • Patchy T2/FLAIR hyperintensities in the subcortical and periventricular white matter
  • Generalized cerebral volume loss with ex vacuo enlargement of the ventricular system
  • Mild mucosal thickening of the paranasal sinuses without fluid levels
  • Secretions pooling in the nasopharynx, which may relate to the indwelling support apparatus

Diagnosis

Artery of Percheron infarct

Sample Report

Areas of acute/early subacute infarct in the right superior cerebellar hemisphere, left greater than right midbrain, and bilateral medial thalami. The thalamic and midbrain infarcts are in the distribution of the artery of Percheron.

No evidence of acute hemorrhage or mass effect.

Remote infarcts in the bilateral cerebellar hemispheres.

Background of chronic small vessel disease and generalized cerebral atrophy.
