Findings

  • Multifocal areas of restricted diffusion along the right greater than left ACA/MCA and MCA/PCA watershed zones
  • Associated T2/FLAIR hyperintensity with mild local mass effect but no midline shift or herniation
  • No acute hemorrhage
  • No hydrocephalus
  • Patchy subcortical and periventricular T2/FLAIR white matter hyperintensities
  • Bilateral mastoid effusions
  • Right pseudophakia

Diagnosis

Watershed infarcts

Sample Report

Acute multifocal infarcts along the right greater than left ACA/MCA and MCA/PCA watershed zones. Mild local mass effect without midline shift or herniation. No acute hemorrhage or hydrocephalus.

Background of chronic small vessel disease.

Bilateral mastoid effusions.

Discussion

  • When you see cerebral infarcts, try to offer a preferred mechanism, as this will help the clinical team determine the next best steps. Here are some broad categories:
    • Thrombotic: within a single arterial territory
    • Embolic: within multiple arterial territories
    • Watershed: concentrated on the borders between arterial territories
    • Venous: within a venous territory
