Findings
- Hyperenhancement and enlargement of the right submandibular gland with surrounding fat stranding and ill-defined fluid
- Multiple small sialoliths (stone 1, stone 2) in Wharton's duct on the right, which is dilated
- Swelling in the right submandibular space exerts mild mass effect on the oropharyngeal airway, which remains patent
- No soft tissue abscess, retropharyngeal edema, or venous thrombosis
- Periapical lucency involving the right mandibular second molar without subperiosteal abscess
- Reactive lymphadenopathy without suppurative changes

Right submandibular sialadenitis with sialolithiasis

Enlarged, hyperenhancing right submandibular gland with surrounding fat stranding, consistent with acute sialadenitis. Multiple small sialoliths are noted in Wharton's duct, which is dilated. Recommend ENT consultation.
