Findings

  • Diffuse gaseous distension of the small and large bowel
  • Linear lucency along the inferior wall of the proximal transverse colon
  • Gastrostomy tube retention balloon overlies the mid upper abdomen
  • Surgical clips overlie the right lower quadrant and anatomic pelvis

Diagnosis

Pneumatosis Ileus

Sample Report

Diffuse gaseous distension of the small and large bowel, which supports the clinical suspicion for ileus.

Linear lucency along the inferior wall of the proximal transverse colon is concerning for pneumatosis. Recommend CT for further evaluation.

Discussion

  • Pneumatosis can be very subtle
  • In this case, the CT confirmed pneumatosis of the proximal transverse colon, but did not show portal venous gas or other signs of ischemia. After workup, the patient was thought to have a non-ischemic cause for pneumatosis
