Findings

Chest radiograph

  • Multifocal patchy airspace opacification throughout both lungs

CT

Chest

  • Widespread bilateral groundglass opacification and more dependent consolidation
  • Intermixed areas of pronounced peribronchial and interlobular septal thickening as well as irregular thick-walled cysts predominantly in the upper lobes

Upper abdomen

  • No acute findings

MSK

  • Generalized muscle wasting

Diagnosis

Pneumocystis pneumonia

Sample Report

Findings are concerning for a diffuse atypical pneumonia, with pneumocystis infection being a primary consideration given imaging appearance and reported history of AIDS.
