Numerous bilateral cavitary pulmonary nodules, for which the primary differential considerations are atypical infection (including tuberculosis), septic embolic disease, autoimmune diseases (including granulomatosis with polyangiitis and rheumatoid arthritis), and cavitary metastases (e.g. thyroid cancer). Given the reported clinical history, atypical infection is the favored diagnosis.
