Findings

Chest radiograph

  • Diffuse interstitial and alveolar opacities
  • Enlarged cardiopericardial silhouette
  • Right IJ approach tunneled dialysis catheter with tip overlying the right atrium

CT

Chest

  • Widespread groundglass opacities with confluent areas of consolidation in the right middle lobe and bilateral lower lobes
  • Nodular lesion in the dependent aspect of the right lower lobe measuring 2.5 x 2.5 cm with internal air-fluid level
  • Diffuse interlobular septal and bronchial wall thickening
  • Moderate-sized right and small left pleural effusions with fluid tracking along the fissures
  • Moderate-sized pericardial effusion
  • Cardiomegaly with notable left ventricular dilation
  • Multiple mildly enlarged mediastinal lymph nodes
  • Hypoattenuation of the blood pool, suggestive of anemia
  • Right IJ approach tunneled dialysis catheter with tip in the right atrium

Upper abdomen

  • No acute findings
  • Multiple calcified splenic granulomas

MSK

  • No acute findings

Diagnosis

Pulmonary abscess

Sample Report

Findings are concerning for panlobar pneumonia with a 2.5 cm right lower lobe pulmonary abscess.

Cardiomegaly with pulmonary edema and moderate-sized right and small left pleural effusions.

Moderate-sized pericardial effusion.
