Findings

Chest radiograph

  • Bilateral interstitial and alveolar opacities

CT

Chest

  • Diffuse interlobular septal and bronchial wall thickening
  • Multifocal consolidation, most concentrated in the dependent aspects of both lungs
  • Moderate-sized bilateral pleural effusions
  • Small pericardial effusion
  • Mildly enlarged bilateral mediastinal and hilar lymph nodes

Upper abdomen

  • Heterogeneous hepatic parenchymal enhancement

MSK

  • No acute findings

Diagnosis

Malarial infection with pulmonary edema, ARDS

Sample Report

Pulmonary edema, likely noncardiogenic related to the patient's diagnosis of malaria. Superimposed multifocal consolidation could relate to ARDS or bacterial pneumonia.

Moderate-sized bilateral pleural effusions.

Small pericardial effusion, which could be seen in the setting of pericarditis.

Heterogeneous hepatic parenchymal enhancement, which could represent congestion or hepatitis.
