Demographics: 44 years old, Female
Indication: Fever, AIDS

Findings

Chest radiograph

  • Mild bilateral hazy opacities
  • Soft tissue density overlying the superior right chest and right shoulder, likely external to the patient

CT

Chest

  • Diffuse centrilobular micronodularity and groundglass opacities with scattered tiny cystic spaces
  • Multiple mildly enlarged mediastinal and bilateral axillary lymph nodes
  • Decreased attenuation of the unenhanced blood pool suggestive of anemia

Upper abdomen

  • Splenomegaly

MSK

  • No acute findings

Diagnosis

Pneumocystis pneumonia

Sample Report

Findings are concerning for a diffuse atypical pneumonia, with pneumocystis infection being a primary consideration in this patient with reported HIV/AIDS.

Multiple enlarged mediastinal and bilateral axillary lymph nodes may be reactive or secondary to the patient's known HIV.

Partially imaged splenomegaly.
