Demographics: 46 years old, Female
Indication: Left flank pain

Findings

Lower chest

  • Mild dependent right lower lobe atelectasis
  • Small pericardial effusion

Abdomen/Pelvis

  • Diffuse left renal parenchymal hypoattenuation with perinephric, peripelvic, and periureteral stranding, urothelial thickening and enhancement, mild hydroureteronephrosis, and gas within the collecting system
  • Patchy areas of hypoattenuation in the right kidney
  • Circumferential bladder wall thickening with intraluminal gas
  • Mildly enlarged left greater than right para-aortic lymph nodes at and just below the level of the renal veins, which are likely reactive
  • Atherosclerotic calcification of the abdominal aorta and branch vessels without aneurysm
  • Bilateral duplicated renal arteries
  • Bilateral tubal ligation clips

MSK

  • No acute findings
  • Mild degenerative changes of the spine

Diagnosis

Emphysematous pyelitis

Pyelonephritis

Sample Report

Findings concerning for bilateral pyelonephritis and left emphysematous pyelitis. No evidence of abscess.

Mild left hydroureteronephrosis.

Bladder wall thickening suggestive of cystitis.
