Demographics: 46 years old, Female
Indication: Right lower quadrant pain

Findings

Lower chest

  • Mild dependent atelectasis

Abdomen/Pelvis

  • Fluid-filled serpiginous tubular structure which extends from the right adnexa along the anterior and superior margins of the uterine fundus with mural hyperenhancement and surrounding fat stranding
  • Peripherally-enhancing, low attenuation structure in the left ovary measuring 5 x 4 cm
  • Small volume free intraperitoneal fluid layering in the anatomic pelvis
  • Subcentimeter hypoattenuating structures in the interpolar right kidney and upper pole of the left kidney, which are too small to characterize
  • Mild dilation and mural thickening of several loops of small bowel in the pelvis
  • Normal appendix

MSK

  • No acute findings
  • Transitional lumbosacral anatomy
  • Bilateral breast implants

Diagnosis

Pyosalpinx

Sample Report

Findings compatible with right hydrosalpinx and possible pyosalpinx given surrounding inflammatory changes. Recommend pelvic ultrasound for further evaluation.

Fluid-attenuation structure in the left ovary, which can also be further evaluated by pelvic ultrasound.

Mild dilation and mural thickening of several loops of small bowel in the pelvis is likely reactive to the above described process.

Normal appendix.
