- Restricted diffusion and T2/FLAIR signal hyperintensity involving the bilateral corpus striatum as well as patchy involvement of the cerebral cortex bilaterally, particularly in the left frontal lobe and left cingulate gyrus
- Additional faint restricted diffusion involving the bilateral thalami
- Dilated perivascular space in the left putamen
- Mild mucosal thickening of bilateral ethmoid air cells

This appearance raises particular concern for Creutzfeldt-Jakob disease (CJD) in the clinical context of progressive neurologic decline.
