患者背景: 5 歳, 女性
適応: 左側伝音性難聴および小耳症

症例 #1

所見

    • 外耳:
      • 軽度の小耳症
      • 外耳道の典型的な位置に厚さ5 mmの固形の骨性閉鎖板を伴う外耳道閉鎖
      • 鼓膜は描出されない
    • 中耳/乳突:
      • 右側に対する左乳突側頭骨の軽度非対称性低含気化
      • ツチ骨柄の形成不全、中鼓室前壁に固着しているように見える
      • キヌタ骨とアブミ骨は正常な外観
      • 中耳の容積と含気は正常
      • 錐体隆起の鈍化した外観と浅い顔面神経陥凹
      • 下降する乳突部顔面神経管の非典型的な分岐で、より大きな管が前方に出ている
    • 内耳
      • IAC、蝸牛、および前庭装置は正常
    • 正常

診断

外耳道閉鎖

主要な画像所見

  • 外耳:
    • 小耳症はしばしばEAC閉鎖を伴うが、臨床的に十分評価されるため、放射線科医が詳細に評価する重要性は低い
    • 正常EACは軟骨部と骨部に分かれ、少なくとも4 mmの直径を有し、通常は外側から内側に向かってわずかに上向きの方向性を示す
    • EAC閉鎖は線維性または骨性であり得る。手術計画の補助のため、閉鎖板の性状と幅を報告することが重要
  • 中耳:
    • 正常中耳は蝸牛岬角から鼓膜または閉鎖板まで少なくとも3 mm必要
    • 耳小骨の様々な異常が見られ、形態異常、耳小骨間の関節異常、中耳壁への固着または耳小骨間の固着が含まれる。アブミ骨は最も影響を受けにくいが、異常アブミ骨の同定は部分的耳小骨置換プロテーゼと全耳小骨置換プロテーゼのいずれを選択するかを決定する主要因子となるため重要
    • 卵円窓と正円窓は通常正常だが、成功のために必要なので評価することが重要
    • 顔面神経の異常走行が一般的で、特に鼓室部が卵円窓上方を下向きに走行することがあり、神経が手術時に損傷するリスクが高くなる。乳突部分枝の異常分岐が見られ、特に下降する乳突部の異常な前方走行は外耳道形成術で損傷され得るため重要
    • これらの患者では先天性真珠腫の発生率が高い

鑑別診断

  • なし

臨床のポイント

  • 外耳と中耳の奇形は共通の胎生学的起源のためしばしば併発する
  • EAC閉鎖の症例では、閉鎖部分が骨性か線維性かを判断する
  • 耳小骨、顔面神経走行(特に鼓室部の下方走行および乳突部の前方走行)、および卵円窓と正円窓の異常を併せて評価し、外科医の支援をする
  • EAC閉鎖患者は先天性真珠腫のリスクが高い

考察

  • 外耳道(EAC)閉鎖は、より広い用語である「先天性耳介閉鎖症」に包含される発達異常のスペクトラムに存在する
  • 第1および第2鰓弓(中胚葉)が中耳および外耳の形成を担うため、EAC閉鎖はしばしば中耳奇形を伴う
  • 対照的に、内耳は耳胞(外胚葉)から発達し、EAC閉鎖の症例では通常正常である
  • EAC閉鎖はしばしば孤立性所見であるが、以下を含むいくつかの症候群を伴って発生することがある:
    • Crouzon症候群(重度の頭蓋縫合早期癒合症および中顔面低形成を想起)
    • Treacher Collins症候群(下顎および中顔面低形成と下顎後退症を想起)
    • Goldenhar症候群(非対称性の小さな半顔面構造および椎骨分節異常を想起)
    • Pierre Robin症候群(小下顎症、舌後退、裂口蓋または高口蓋を想起)
  • EAC閉鎖は感音性ではなく伝音性難聴を生じる
  • 管理選択肢には以下が含まれる:
    • 内耳への音伝導を改善するための骨固定型補聴器
    • 耳小骨連鎖再建の有無にかかわらず、外耳道形成術および鼓膜形成術
