Demographics: 11 years old, Male

Findings

  • No acute fracture or malalignment
  • Bubbly lucent lesion in the proximal tibial diaphysis measuring 5.5 x 2.7 cm with a narrow zone of transition and no periosteal reaction. The lesion involves greater than 50% of the tibial diameter

Diagnosis

No acute fracture or malalignment.

Bubbly lucent lesion in the proximal tibial diaphysis measuring 5.5 x 2.7 cm with a narrow zone of transition and no periosteal reaction. This lesion has non-aggressive features with differential diagnosis including aneurysmal bone cyst, multilocular bone cyst, and nonossifying fibroma. MRI could further evaluate if clinically indicated. The lesion involves greater than 50% of the tibial diameter, which places the patient at increased risk for pathologic fracture, though no fracture is identified.
