Gradual onset bilateral leg weakness over several months

Findings

  • Thickening and enhancement involving nearly all of the nerve roots of the cauda equina, most pronounced inferior to the level of L3-L4
  • Normal position, signal, and contour of the conus
  • Mild multilevel degenerative changes, most advanced at L5-S1 where there is disc dessication and height loss with a broad-based disc bulge. No significant spinal canal or neural foraminal stenosis

Diagnosis

Chronic inflammatory demyelinating polyneuropathy (CIDP)

Sample Report

Thickening and enhancement involving nearly all of the nerve roots of the cauda equina, which is most consistent with chronic inflammatory demyelinating polyneuropathy (CIDP) given the reported gradual onset. Recommend correlation with CSF analysis.
