Findings
- Areas of confluent T2/FLAIR signal hyperintensity in the left frontal and parietal white matter and to a lesser extent in the right frontal, right parietal, and bilateral occipital lobes with involvement of the subcortical U fibers
- No associated restricted diffusion or enhancement
- No evidence of acute hemorrhage, mass effect, or hydrocephalus
- Extensive mucosal thickening of the paranasal sinuses
Diagnosis
Progressive multifocal leukoencephalopathy (PML)
Sample Report
Asymmetric areas of confluent T2/FLAIR signal hyperintensity in the left greater than right cerebral white matter without associated restricted diffusion, enhancement, or mass effect. Given patient history, this imaging appearance is most suggestive of progressive multifocal leukoencephalopathy (PML).
Extensive mucosal thickening of the paranasal sinuses, which can be seen with acute sinusitis.
