Demographics: 41 years old, Male
Indication: Headache

Findings

When compared to the initial CT:

  • The follow up at 2 days shows subtle decrease in size of the lateral, third, and fourth ventricles. There also has been interval crowding of the subarachnoid spaces and mild effacement of the basal cisterns
    • Gray/white differentiation is preserved
    • No herniation or midline shift
    • No acute hemorrhage
  • The follow up CT at three days shows further progression of cerebral edema with severe diffuse sulcal and cisternal effacement and compression of the ventricles.  There is crowding at the foramen magnum without gross evidence of herniation

Diagnosis

Diffuse cerebral edema

Day 3 study: Further progression of cerebral edema with severe diffuse sulcal and cisternal effacement and compression of the ventricles. There is crowding at the foramen magnum without evidence of frank herniation or hemorrhage.
