Demographics: 66 years old, Female
Indication: Chest pain

Findings

Chest radiograph

  • Enlarged cardiopericardial silhouette with calcification in the left ventricular wall centered around the apex

CT

Chest

  • Ballooning of the left ventricular apex and apical anterolateral wall with mural calcification and partially calcified mural thrombus
  • Thoracic aortic atherosclerosis without aneurysm
  • Moderate coronary artery calcification
  • Small hiatal hernia
  • Multiple pleural based nodules in the left hemithorax, some of which have associated calcification

Upper abdomen

  • No acute findings
  • Cholecystectomy

MSK

  • No acute findings

Diagnosis

Left ventricular true aneurysm

Sample Report

No acute findings.

Ballooning of the left ventricular apex and apical anterolateral wall with mural calcification and associated partially calcified thrombus at the apex, most suggestive of a true aneurysm.

Multiple partially calcified pleural-based nodules in the left hemithorax. Recommend correlation with prior outside imaging, if available, to document stability. Otherwise consider follow-up chest CT in 3 months or PET/CT for further evaluation.
