Indication: Lower abdominal pain, leukocytosis

Findings

Lower chest

  • No acute findings
  • Moderate coronary artery calcification

Abdomen/Pelvis

  • Complex, peripherally-enhancing fluid collection in the prostate gland measuring 6.5 x 5.5 x 4.5 cm, which bulges posteriorly contacting the rectum
  • Rightward displacement of the prostatic urethra
  • Rounded hypoattenuating structure along the inferior right aspect of the prostate gland measuring 1.7 x 1.2 cm
  • Mild circumferential mural thickening of the urinary bladder
  • Cholecystectomy
  • Appendectomy
  • Atherosclerotic calcification of the abdominal aorta and branch vessels without aneurysm

MSK

  • No acute findings
  • Surgical changes of L4-S1 posterior instrumented spinal fusion
  • Bone graft harvest site in the posterior right iliac bone
  • Grade 2 anterolisthesis at L5-S1
  • Scaphoid abdomen

Diagnosis

Prostatic abscess

Sample Report

Complex, peripherally-enhancing fluid collection in the prostate gland measuring up to 6.5 cm, which is concerning for prostatic abscess. Although considered less likely, a necrotic prostatic neoplasm could have a similar imaging appearance. Recommend evaluation by urology.

Rounded hypoattenuating structure along the inferior right aspect of the prostate gland measuring up to 1.7 cm which may represent adjacent reactive fluid, though a necrotic lymph node could have a similar appearance.

Mild circumferential mural thickening of the urinary bladder, which may relate to cystitis or bladder outlet obstruction.
