Demographics: 62 years old, Male
Indication: Neutropenic fever

Case #15

Findings

Lower chest

  • Mild cardiomegaly
  • Low attenuation of the unenhanced blood pool suggestive of anemia
  • Mild dependent atelectasis
  • Trace pleural effusions

Abdomen/Pelvis

  • Mural thickening of the cecum, ascending colon, and terminal ileum with surrounding fat stranding
  • Small volume free fluid layering in the anatomic pelvis
  • Hepatomegaly with diffusely decreased attenuation of the hepatic parenchyma
  • Bilateral renal cortical scarring and nonspecific mild perinephric stranding
  • Multiple mildly enlarged ileocolic lymph nodes, likely reactive
  • Mild atherosclerotic calcification of the abdominal aorta and branch vessels without aneurysm
  • Prostatectomy

MSK

  • No acute findings
  • Multiple sclerotic foci throughout the axial skeleton
  • Partially imaged median sternotomy changes and remote healed upper sternal body fracture

Diagnosis

Typhlitis

Sample Report

Inflammatory changes of the cecum extending to the terminal ileum and ascending colon, concerning for typhlitis in this neutropenic patient.

Small volume free fluid layering in the pelvis without evidence of intraabdominal abscess.

Hepatomegaly with hepatic steatosis.
