Findings
- Right orbital floor blowout fracture with herniation of orbital fat through the fracture defect. Fracture extends inferiorly through the anterior and posterior walls of the right maxillary sinus with involvement of the infraorbital foramen and right maxillary hemosinus
- Acute fractures of the left pterygoid plates and of the posterior wall of the left maxillary sinus
- Minimal fat stranding in the retrobulbar, intraconal right orbit near the optic nerve insertion site
- Extensive facial soft tissue swelling with multiple radiopaque subcutaneous foreign bodies overlying the right zygomatic arch
- Numerous dental caries and periapical lucencies. Inflammatory mucosal thickening of the paranasal sinuses is likely odontogenic in origin
Diagnosis

Sample Report
Right orbital floor blowout fracture with herniation of orbital fat through the fracture defect. Fracture extends inferiorly through the anterior and posterior walls of the right maxillary sinus with involvement of the infraorbital foramen and right maxillary hemosinus.
No imaging evidence of extraocular muscle entrapment. Minimal fat stranding in the retrobulbar, intraconal right orbit near the optic nerve insertion site, which may represent trace hemorrhage. No proptosis. Globes are intact.
