Case #11


Findings

  • Acute mildly displaced C1 burst fracture (Jefferson fracture) with fracture of the anterior arch centrally and fracture of the posterior arch on the right. Additional mildly displaced avulsion fracture from the medial aspect of the left lateral mass of C1
  • Mild anterior subluxation of the left lateral mass of C1 with respect to the occipital condyle
  • Mild subluxation and anterior widening of both C1-C2 articulations
  • Acute type II odontoid fracture with posterior angulation of the odontoid fracture fragment
  • Mild stepwise anterolisthesis from C3-C4 through C5-C6
  • Multilevel degenerative changes in the cervical spine, with most severe disc space narrowing at C6-C7 and erosive facet arthropathy on the left at C5-C6


Diagnosis

  • Jefferson fracture
  • Type II odontoid fracture
  • Possible transverse ligament avulsion fracture

Sample Report

Acute mildly displaced C1 burst fracture (Jefferson fracture) with fracture of the anterior arch centrally and fracture of the posterior arch on the right. Additional mildly displaced avulsion fracture from the medial aspect of the left lateral mass of C1 at the site of transverse ligament attachment. MRI could further assess the degree of ligamentous injury.

Mild anterior subluxation of the left lateral mass of C1 with respect to the occipital condyle. Mild subluxation and anterior widening of both C1-C2 articulations.

Acute type II odontoid fracture with posterior angulation of the odontoid fracture fragment.

Mild stepwise anterolisthesis from C3-C4 through C5-C6, which is likely degenerative in etiology. However, MRI could further assess for ligament injury if clinically warranted.
